Staff Hiring Forms

Notice to All Applicants:
The YMCA enforces its policies and practices to prevent child abuse. Any potential employee has to pass a thorough local and national background check, as well as reference checks (professional and personal).  We have strict abuse reporting procedures, required trainings and a code of conduct for staff. We minimize opportunities for abuse to occur and we talk with children about personal safety and touching limits.

Follow the steps below to complete the application process.

STEP 1: DOWNLOAD, PRINT AND FILL OUT AN EMPLOYMENT APPLICATION 

Once you have been offered a position at the West Suburban YMCA, you will need to complete the following steps.

STEP 2: DOWNLOAD, PRINT AND FILL OUT THE FOLLOWING FORMS
    •    Signed Offer Letter if applicable (mailed/emailed to you)
    •    Signed Job Description (mailed/emailed to you)
    •    CORI (bring photo ID)
    •    W-4
    •    I-9 (present proper IDs to hiring manager)
    •    Direct Deposit Form
    •    Work Permit (required for staff 17 & under)
    •    Equal Opportunity Information
    •    Membership Application

STEP 3: SUBMIT THE FOLLOWING ONLINE FORMS

STEP 4: COMPLETE YOUR STAFF TRAININGS
    •    Online staff training/reading (the link, username and password will be emailed to you from HR)
    •    Contact Phil Wong to schedule your CPR and First Aid training
    •    Attend New Employee Orientation (an invitation/schedule will be emailed to you from HR)
